Easy question: General/Classic Arcade Games: Although most classic arcade games were meant to be endless due to programming errors many of them reach a point where they become unplayable. For example in Pacman you reach a stage where half the screen becomes jumbled, and in Donkey Kong you eventually reach a stage where Mario/Jumpman will just die after a few seconds. What is the common name for these stages?

That's a touggh question, actually. Do you count the various releases of the N64 cart? I think there are 3 different ones (The original gold cart, the second release on a standard grey cart, and then a third bugfix release). There is also the Master Quest GCN promotional disc that came with Wind Waker (is this one version, or two different ones on the same disc, I wonder?), and the Zelda collection GCN promotional disc that came with the platinum GCN package. You can also download the game for the virtual console on the Wii. To top it off (though I don't think this counts), you can also play a timed demo of the game on Super Smash Bros. Brawl as a masterpiece.
So, it all depends on your interpretation. At minimum, there are 4 different versions (N64 cart, Master Quest, Zelda Collection, and Virtual Console). If you count the multiple cart versions, there are 6 (add the two updates to the list for N64). And if you count the masterpiece from Brawl, there are 7.
I think it's time to retire the SMRPG question. Why does nobody know this? At the very least, everyone should have downloaded the game for their Virtual Console by now; it was released a few weeks ago.
The answer is that Samus and Link each make a cameo. Link even makes his "found a secret" noise when you talk to him!

The (I believe) octorok in the pictobox mini-game/sidequest thing is captioned with "Perfect Attendance" because it's been in each Zelda game? Except not in Twilight Princess, unless they really changed the design of it and I didn't notice.

+1 to DjayD although, I might point out my original posting about the cigar flicking cat wasn't a trivia question, rather I just made a post about how awesome it was.
Also I might add good job on the Zelda question, I would have left out the bug fix cart.
As for Super Mario RPG, downloading the game for Virtual Console has definitely been on my to do list; its one of those game's I've never played, but really should have. If I could ever finish reading my Set Theory book I could actually justify getting it. :shakes fist at summer goals:
I'm leaving the classic arcade question open for another few days, but I'ma try and think of a new one to keep things going.
