The Halloween season is making me want to play VTMB again. Though it isn't specifically halloween-related, it's got one of the most suitable atmospheres of any game I own.

Out with the old, in with the new.
Dragon Quest 8 and Dragon Quest (Warrior) 3 are connected by the presence of Empyrea / Ramia, the giant bird. You summon her and use her to fly in Dragon Quest 3, and she grants you an item that allows you to fly (or is at least heavily involved in the plot relevant to said item) in Dragon Quest 8.
Dragon Quest 8 and Dragon Quest (Warrior) 4 are connected by the presence of Ragnar and Taloon. They are both party members in Dragon Quest 4, and both appear as rival monster trainers in the arena in Dragon Quest 8.
Your reward for maxxing out your overall Star Piece count in Super Mario Galaxy is the transformation of all the coconuts in the game to watermelons. Yeah, that's all it does.

Well, it's been a week. I'm going to dump the question and post a new one... but it sure looks like the topic is dead.
The Rocket Launcher and Flamethrower can make instant kills.

Super Mario World (SNES): A 100% clear save file is listed as having opened all 96 stage exits. What, then, is the significance of the 92 challenge?
Bonus point to anyone who was crazy (like me) enough to actually complete said challenge! Of course, in order to get that point, you have to know what the challenge is about first...

As for SMW SNES, perhaps there's no excuse for having not played it, but what about not being fanatical enough to be privy to these alleged "challenges" ?
I'm going to guess that it has something to do with beating the game while skipping a few "essential stages" using alternate routes and stuff.

You would be correct about skipping stages. A quick think about the actual stages and which ones would be missing should bring up a pretty obvious answer.
However, I'm just glad someone actually still cares about posting, so have a point on the house.

Those are the only 3, which is why you don't remember any others. I await N3B's confirmation of that answer, though.
The 92 challenge in Super Mario World involves clearing every stage exit except for the 4 "!" switches. Since clearing these causes platforms of the same color to appear in strategic locations throughout the game, it tends to make life difficult in more than a few places.
The water caravan in Fallout is a double edged sword because hiring them grants you a bit of short term relief in exchange for long term safety. Specifically, hiring them will extend the time limit on the Water Chip quest (which is the initial goal of the game). In exchange for that, however, the time limit for the actual main quest, stopping the Super Mutants, is reduced since you revealed the secret location of Vault 13 to outsiders.

Didn't know then and I don't know now.
In Zelda 64: The Quest for the Golden Tunic, better known as TLOZOOT, I believe 5 characters die in scripted events (excluding bosses and enemies etc). Two of them we actually see die, and the other three we find out about post-mortem. There may be one or two others that I overlooked. Any characters that simply disappear between Present World and Future World, such as the Magic Bean Dealer, are presumed to have been safely relocated and to not have died.

Well, hey. Nobody had answered a question in weeks, so I just let it drop. Now that you're all back, though...
The book of the dead for Ocarina, eh? Hmmm...
I believe Ganondorf kills the King during his conquest of Hyrule during the 7 year gap. That blue guy whose name I can't remember (from the Cucco part of the Biggoron Sword quest, He's Anju's brother) is "turned into a skull kid" apparently, which means he's pretty much dead. The Deku Tree dies shortly after you complete the dungeon inside him. Dampe the gravekeeper dies sometime during the 7 year gap, seeing as how you encounter his ghost. Link's father is slain in battle, and his mother dies in Kokiri Village, before the start of the game. I'm not sure if they count, since you only find that out from a flashback. Lord Jabu-Jabu is presumably killed by Ganondorf, seeing as how Zora's realm and his lake are partially frozen over in the future. I'm not sure if it's actually explicitly stated that he dies, however. Kotake and Koume both inexplicably ascend to heaven during a cutscene, though I'm assuming they don't count since they are collectively the Twinrova boss.
I can't think of anyone else, though I'm probably missing at least one of the characters you had in mind.
The running theme number in Eternal Darkness is 333, or perhaps more simply, just 3.
